Mental Health Cooperative, Inc. (MHC) was formed in 1993 to serve individuals with severe and persistent mental illness. Since then, we have expanded our services to children and adolescents with severe emotional disorders across Middle and East Tennessee.
Our sole purpose is to support and treat those challenged with serious mental illness and poverty. Although based out of Nashville, we serve several communities across middle and East Tennessee with satellite offices in Antioch, Gallatin, Dickson, Columbia, Cleveland, Murfreesboro, Clarksville, Cookeville, Chattanooga, and Memphis.

The Care Coordinator provides targeted support services, via the Wraparound Process, to families with high risk young children/children/youth/transition-age youth, regarding behavioral or emotional problems, parenting issues, and life stressors by developing and implementing bio-psycho-social assessments and service plans with the family and wraparound team members. The Care Coordinator, utilizing the System of Care (SOC) philosophy will work with families, social service organizations, and other state agencies to develop a coordinated network of resources and supports in their communities.

Schedule: Mon-Fri, 8a-4:30p
Salary: $41,000 (base pay) with additional incentive for language skills and behavioral health experience. Mileage reimbursement available.

Summary: The Care Coordinator provides targeted support services, via the Wraparound Process, to families with high risk young children/children/youth/transition-age youth, regarding behavioral or emotional problems, parenting issues, and life stressors by developing and implementing bio-psycho-social assessments and service plans with the family and wraparound team members. The Care Coordinator, utilizing the System of Care (SOC) philosophy will work with families, social service organizations, and other state agencies to develop a coordinated network of resources and supports in their communities.
Responsibilities:
Ensure all phases and activities of the Wraparound Process are done with high quality, fidelity, and accountability to family team.
Provide outreach and engagement to families upon referral to the program by conducting initial phone contact and the assessment process with the assistance of the Family Support Specialist (FSS).
Complete an assessment of the family, including Child and Adolescent Needs and Strengths (CANS) assessment, Caregiver Strain assessment, and other national evaluation tools to develop a care plan.
In partnership with the FSS and therapist, provide face-to-face direct service hours to families including support, referrals, and assistance with the development of positive parenting, crisis planning, basic life skills, and parenting a child/youth with special needs.
In collaboration with the FSS, assist family in developing a list of individuals they want to attend the family team meetings.
Assist in facilitation of the family team meetings:
Develop agenda with caregiver; lead the meeting with caregiver until the caregiver is ready to do so alone.
Arrange for time and place of meeting.
Ensure that family, team and other required paperwork are completed as instructed.
Complete and disseminate service plan to team members with their tasks highlighted.
Schedule subsequent family team meetings and provide members with date, time, and location.
Partner with family/team members in identifying and overcoming barriers/service gaps
Ensure the Wraparound Process is family driven and that families experience ongoing participation in/ownership of their care plans.
Provide liaison functions for the team members and service providers.
Create family team and schedule case conferences/family team meetings in collaboration with FSS and family members.
Because of the family team process, develop and coordinate Master Treatment Plan (MTP) with the family and FSS.
Linkage to community resources, clinical services, and natural supports related to the child/youth.
Be available for crisis calls; collaborate with clinician/therapist to provide support during and following the crisis.
In consultation with therapist, ensure that crisis plan, and assessment are completed.
Skills building for the child/youth as appropriate.
Maintain up-to-date case records on each client family, including weekly contact reports, service agreements, evaluations, and termination summaries.
Maintain and build community relations through education, advocacy, and participation in external meetings such as the Community Action Board (CAB), Council on Children s Mental Health (CCMH), and other meetings related to children/youth and families.
Collaborate with program supervisor to train staff; provide field case consultation and support to new staff.
Participate and engage in the local SOCAT team.
Work with the SOCAT Regional Coordinator to identify and engage with local community resources.
Provide SOC 101, wraparound orientation and training, and other pertinent trainings to new staff and other community partners.
REQUIREMENTS:
Bachelor s degree in a social or behavioral science or other related acceptable field; One to three years social services training/experience.
Previous experience working with children/youth and families with significant behavioral health needs preferred.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
